Evenflo Switch-A-Roo Telephone Toys, made between October 2008 and June 2009, model number 6391911 has a mirror decal attached to the toy that can peel away, posing a potential choking hazard. **Kolcraft Recalls 1 Million Play Yards Due to Fall Hazard

The play yard’s side rail can fail to latch properly and when a child pushes against the rail it can unlatch unexpectedly, posing a fall hazard to children. The firm has received 347 reports of sides of the play yard collapsing unexpectedly, resulting in 21 injuries to young children, including bumps, scrapes, bruises and one concussion. This recall involves the Kolcraft Travelin’ Tot play yards, as well as other similar play yards manufactured for Carter’s, Sesame Street, Jeep, Contours, Care Bare and Eric Carle, with model numbers listed on website cited below. Some of the units have a bassinet and/or changing table. Some models have a mobile, vibrating unit or a parent organizer. Model numbers are printed on a white sticker on one of the feet of the play yard. **Aqua-Leisure Industries Recalls Inflatable Baby Floats Due to Drowning Hazard

The leg straps in the seat of the float can tear, causing children to unexpectedly fall into or under the water, posing a risk of drowning. There have been 31 reports of float seats tearing, causing children to fall into or under the water. No injuries have been reported. The recalled inflatable baby floats were sold in a variety of styles and colors. The names and model numbers are located on the packaging. A date between 08/02 and 07/08 is molded on the valves of recalled products. Products not included in the recall can be identified with the lettering “NP” molded on the valve. **Toy Maracas Recalled by Tupperware U.S. Due to Choking and Suffocation Hazards

The maracas can break and expose small parts, posing a choking hazard to young children. In addition, the handle poses a suffocation hazard to young children. This recall involves blue and red toy maracas with yellow handles. The maracas were used by Tupperware representatives during sales presentations. Cut-outs in the shape of stars, squares and other geometric figures are located inside the maraca. **Simplicity Play Yards Recalled by Various Retailers Due to Fall and Entrapment Hazards

One or more rails can collapse unexpectedly, posing a fall or entrapment hazard to young children. The recalled play yards are portable and were sold with a bassinet, changing table and mobile features. The play yards bear the “Simplicity” logo. The model numbers are 5500DRM, 5500WDS, 5500FEL, 5501FEL, 5502MON, 5520PRO, 5550HAN, 5700MAN, and 5750MIR. The model number is located on a sticker on one of the legs underneath the play yard.

**Pacifiers Recalled by Healthtex Due to Choking Hazard

The pacifiers failed to meet federal safety standards. The nipples can separate from the base easily, posing a choking hazard to young children. This recall involves Zoo pacifiers with animal cartoon figures as handles. The pacifier has a round-shaped mouth guard with three ventilation holes. “Made in Spain” is printed on the handle side of the mouth guard and “Mi Tosoro” is printed on the cardboard packaging. **Fisher-Price 3-in-1 High Chairs
The seat can fall backwards from high chair frame if the booster seat release is unlatched while the child is in the product. Also, the seat back can detach if not fully snapped in place, posing a fall hazard and risk of serious injury to young children. The firm has received one report of a seat back detaching and child falling out, resulting in a skull fracture.
Description: This recall involves the 3-in-1 High Chair to Booster™, which converts from a high chair to a toddler booster seat. It includes a removable tray, height adjustment and folds for storage. The product number (P5369) is printed on the side of the seat, on a label on the seat pad, and on the product’s packaging. Sold exclusively at: Target stores nationwide from December 2008 through March 2009 for about $100 and Manufactured in Mexico.

**Evenflo Envision High Chairs
Recline fasteners and metal screws on both sides of the high chair can loosen and fall out, allowing the seatback to detach or recline unexpectedly. Children can fall backwards or fall out of the high chair and suffer bumps and bruises to the head, abrasions, cuts and bruises. Detached hardware also poses a choking hazard to children. Evenflo has received 320 reports of seatbacks detaching or reclining unexpectedly, resulting in 19 reports of bumps and bruises to the head and 35 reports of other injuries including abrasions, pinches and bruises. Evenflo has also received 13 reports of recline fasteners and screws falling out of the high chair, resulting in one incident in which a fastener and/or screws were found in a child’s hand or mouth but were removed before choking occurred.
The recall involves all Evenflo Envision™ high chairs, including model numbers: 2891321, 2891321A, 2891333, 2891351, 2891351A, 2891365, 2891375, 2891403, 2891403A, 2891466, 2891466A, 2891478, 2891536, 2891536A, 2891573, 2891586, 2892351 and 2892351A. **Evenflo Majestic High Chairs
Plastic caps and metal screws on both sides of the high chair can loosen and fall out, allowing the seatback to suddenly fall back or detach from the high chair. Children can fall backwards or fall out of the high chair and suffer bumps and bruises to the head, broken bones, abrasions, cuts and bruises. Detached hardware also poses a choking hazard to children. For the models added by this expanded recall, Evenflo has received 16 reports of seatbacks reclining, falling back and/or detaching unexpectedly, resulting in five reports of bumps and bruises to the head and five reports of other injuries including bruises and abrasions. For the models added by this expanded recall, Evenflo has received 145 reports of plastic caps and screws falling out of the high chairs, including one incident in which a cap and/or screws were found in a child’s hand or mouth but were removed before choking occurred.
The expanded recall involves Evenflo Majestic™ high chairs, model numbers: 3001395, 3001395A, 3001604, 3001700, 3001713, 3001713A, 3001730A, 3001732, 3001733, 3001742, 3001742A, 3001756, 3001821A, 3001840A, 3001845, 30011848, 3001852, 3001932, 3002719 and 3003845 manufactured between January 23, 2007 and December 31, 2008. **Mylicon Gas Relief Dye Free Drops

**Playhouse Disney “Handy Manny” Toy Tool Sets
The tools contain eyes that can separate, posing a choking hazard to young children. There have been three reports of detached eyes, including two reports of children who started to choke on the pieces. No injuries have been reported. The recalled Playhouse Disney “Handy Manny” Tool Set is a plastic, eight-piece toy set that includes a tool box, hammer, saw, pliers, wrench, tape measure and two screwdrivers. **Safety 1st SmartLight Stair Gates
Dorel Juvenile Group USA, of Columbus, Ind., is voluntarily recalling 100,000 Safety 1st SmartLight Stair Gates. The hinges that hold the stair gate in place can break, posing a fall hazard to children if the gate is placed at the top of the stairs. The recall involves the Safety 1st SmartLight Stair Gate with model number 42111, which is printed on a sticker under the handle panel. The gates, which were manufactured before December 31, 2007, are white metal with a gray handle and a motion sensor nightlight that illuminates on approach. The manufacture date is located below the gate’s handle as a date wheel imprinted in the plastic; an arrow indicates the month, and the year is stamped inside the circle. The manufacture date is also printed on a sticker inside the battery door. **Britax Frontier Car Seats Recalled Due to Injury Risk
Britax recalled an undisclosed number of Frontier car seats (manufactured from April 1, 2008, through September 14, 2008, and sold at mass merchandisers and independent juvenile retailers) because the harness straps can detach from the car seat, which could cause an increased risk of injury in the event of a crash. This recall involves Britax Frontier that have the following model numbers and model names: E9L54E7 (Red Rock), E9L54H6 (Rushmore), E9L54H7 (Pink Sky), and E9L54M6 (Canyon). Model E9L5490 (Sahara) was also recalled and was manufactured from April 1, 2008, to September 17, 2008. The date of manufacture and model number may be found on the white label located near the top of the car seat on the child's left side under the cover. **24,000 Newborn and Infant Pants from JCPenney Recalled Due to Choking Hazard
JCPenney has recalled about 24,000 newborn and infant pants (sold at JCPenney stores nationwide and at www.jcp.com from July 2008 through October 2008) after discovering that the metal snap at the waist can detach and pose a choking hazard to infants. The recall involves Arizona brand newborn and infant girls' and boys' denim pants. They were sold in sizes 0-3 months, 3-6 months, and 6-9 months. Only those pants made in Bangladesh are affected; pants made in China are not included in the recall. The country of origin is located on a label sewn into the waistband.
